Die decima quarta decembris baptizavi sub conditione Christophoram (in partu ab obstetrice Joanna S[?]ys baptizatam) filiam Henrici V[...] et Adrianae [...] coniugum [...].

Translation

On 14 December [1679] I baptised conditionally Christophora (baptised at the birth by the midwife Joanna S?ys), daughter of Henricus V[...] and Adriana [...], a married couple [...].
